We are sorry about your experience and we’re glad to have this opportunity to make amends. To help us fix things, we will need some information about the product you purchased. This form is set up to collect information for a single product you found less than euphoric. If you experienced problems with more than one product, please fill out a separate form.

Container Info
Next, on the bottom of the container you will notice a product manfacturing code. We need this information to help us identify the product you purchased and where and when it was made. This code can be in a couple of different formats so we’ve shown some examples to help you fill out this form. Now you’ll know what that silly code on the bottom of our containers means!

Plant Number:

The 2 or 3 digit number that appears on the second line after "50" as shown in the top example found on the right

OR

the 2 or 3 digit number that appears on the second line after "14" as shown in the bottom example found on the right.

The 5 digit number at the end of the second line as shown in the top example found on the right

OR

the 5 digit number immediately preceding the "00" at the end of the second line as shown in the bottom example found on the right.

Expiration Date:

The expiration date is the date on the first line as shown in both examples found on the right, it uses the mon-dd-yyyy date format.

Production Line:

The production line is a 1 character letter or number. It is the first character after the plant number as shown in the top example found on the right

OR

the first character after the "SP" as shown in the bottom example found on the right.

Manufacture Time:

The time that the product was manufactured (hh:mm) is located on the second line as shown in both examples found on the right.
